Scheduling an Appointment with HeartSavers Clinics

Cardio 101 Appointments

Appointment times available​

 

First Time Patients:  Call 612-913-9758 to make an appointment.   

• After scheduling, you'll be given the option to complete all intake forms online through our secure medical record system to save time upon arriving at the clinic (we will send you a link to set up your Patient Portal -- where forms will be waiting for you to open, read, and e-sign.)

Returning/Previous HeartSavers Clinics Patients: 

• If you have not been seen at our Plymouth MN location, please follow the First Time Patients process above.  

• After scheduling, you'll also be given the option to complete all intake forms online through our secure medical record system to save time upon arriving at the clinic (we will send you a link to set up your Patient Portal -- where forms will be waiting for you to open, read, and e-sign.) 

        (We are asking all returning patients to update their medical history and personal information for our new

         electronic health record system)

Returning Patients (after having a 2023 appointment): 

• Once you have been seen at HeartSavers Clinics Plymouth MN after the start of 2023, you will have the option of logging into your HeartSavers Clinics Patient Portal to make appointments.

• You can also use your Patient Portal to confirm a future appointment you have scheduled. 

• Alternatively, call (612) 913-9758 for scheduling.
