Scheduling an Appointment with HeartSavers Clinics

First Time Patients:

When appointments are re-started, you'll be given the option to complete all intake forms online through our secure medical record system to save time upon arriving at the clinic (we will send you a link to set up your Patient Portal -- where forms will be waiting for you to open, read, and e-sign.)

Returning/Previous HeartSavers Clinics Patients:

If you have previously been seen elsewhere, please follow the First Time Patients process above.

(We are asking all returning patients to update their medical history and personal information for our new

electronic health record system)

Information regarding insurance:

We currently are not covered by commercial insurance or Medicare
We are in the process of credentialing with payers (including Medicare)
Therefore, the Cardio 101 is being charged at $125
This amount can be paid by cash, check or credit card
For those wanting to proceed to the Cardio 1000 (prior to insurance being accepted) the cost is $1,375
