Cardio 101 Appointments

First Time Patients: Call 612-913-9758 to make an appointment.

After scheduling, you'll be given the option to complete all intake forms online through our secure medical record system to save time upon arriving at the clinic (we will send you a link to set up your Patient Portal -- where forms will be waiting for you to open, read, and e-sign.)

If you have not been seen at our Plymouth MN location, please follow the First Time Patients process above.

Returning Patients (after having a 2023 appointment):

Once you have been seen at HeartSavers Clinics Plymouth MN after the start of 2023, you will have the option of logging into your HeartSavers Clinics Patient Portal to make appointments.

You can also use your Patient Portal to confirm a future appointment you have scheduled.

We currently are not covered by commercial insurance or Medicare

We are in the process of credentialing with payers (including Medicare)

Therefore, the Cardio 101 is being charged at $125

This amount can be paid by cash, check or credit card

For those wanting to proceed to the Cardio 1000 (prior to insurance being accepted) the cost is $1,375
