Notice of Privacy Practices and Consent to Use Data for Research

All patients coming to a HeartSavers Clinics appointment for the first time will complete normal / typical forms that include such things as: 

• Insurance form

• Medical history

• Patient financial responsibility (in the event your insurance does not cover our services)

• HIPAA (privacy) form 

• Notice that you will receive our Notice of Privacy Practices and Consent to Use Data for Research  (It is important for cardiovascular disease research to be done to help advance future evaluation and treatment opportunities.  We will only ever use anonymized patient information.  The form you will receive at the clinic (and is available through this web page) explains these practices and the consent you will give.  At any time, you may opt out of consent.
